Inspector Notes
An unannounced monitoring inspection was conducted on 07/05/2022. There were 59 children, ages 2 months- 6 years, 11 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed in free choice activities, outdoor time, teacher directed activities and lunch time. The inspector discussed the following with the provider: cleaning and sanitizing, children?s records, staff records and background check process. A total of 5 children?s records, 4 staff records and 2 medications were reviewed. The inspector arrived for the inspection at 10:15 am and departed at 12:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
2Evidence: The blue flooring on the bridge and slide enclosure of the 2 Step plastic play structure was cracked; the cracks in the plastic material created a pinch point.
Evidence:
1. The attendance record in the ?Pre-K? room indicated 14 children present. There were 18 children present at the time of inspection. The attendance did not document the arrival as it occurred.
2. The attendance record in the ?16 month-24 month? room indicated 5 children present. There were 6 children present at the time of inspection. The attendance did not document the arrival as it occurred.
3. The attendance record in the ?Infant? room indicated 8 children present. There were 9 children present at the time of inspection. The attendance did not document the arrival as it occurred.
4. The attendance record in the ?2 year old? room indicated 11 children present. There were 12 children present at the time of inspection. The attendance did not document the arrival as it occurred.
