Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/29/2024 and completed on 07/29/2024. The on-site inspection began at 12:30 pm and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 35 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence: The record for Staff 1 contained a documented hire date of 05/31/2024. The date on the tuberculosis screening in the record was 06/13/2024. The documentation was not received at the time of employment.
Plan of Correction: Provider will obtain TB screening prior to hire.
