serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site Renewal Inspection was initiated on August 26, 2025 and completed on August 26, 2025. The on-site inspection began at 1:00 pm and ended at 3:30 pm. The inspector reviewed compliance in the areas listed above. There were 45 children and 5 staff
present during the inspection. An additional staff member arrived during the inspection. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 5, 2025. A POC submitted after this date will not appear on the public website.
Violations
2One Staff Member was observed sitting in the hallway between two classrooms during rest period. There were ten 2-year old children in one classroom and ten 3-year old children in the classroom directly across the hallway. Staff Member was not within sight and sound of either classroom and was not in the same space as the resting or sleeping children.
In the infant room refrigerator, there were 3 infant bottles containing prepared infant formula, that were not dated nor labeled with the child's name.
