Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 6/1/21 and concluded on 6/9/21. The director was contacted by telephone to initiate the inspection. There were 88 children present and 17 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children's records and 5 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
3Evidence:
1. There was no record of an emergency evacuation drill for the months of October 2020 and March 2021.
2. The director confirmed that emergency evacuation drills were not conducted for the months of October 2020 and March 2021.
Evidence:
1. The record for child 1 (date of enrollment: 5/3/21) contains only one name, address, and phone number of a designated person to call in an emergency if a parent cannot be reached, where two are required.
2. The center director confirmed that the record for child 1 is lacking one of the required two emergency contacts.
Evidence:
1. Staff 1 has a hire date of 3/22/21. Staff 1 indicated on her sworn statement or affirmation that she has resided in the state of Alabama within the past five years. The results of a sex offender registry check and a child abuse and neglect registry check were not available for staff 1 from the state of Alabama.
2. The center director confirmed that the sex offender registry check and the child abuse and neglect registry check for staff 1 were not obtained from the state of Alabama.
