Inspector Notes
A monitoring inspection was conducted on 4/29/2022 from 10:20am until 1:53pm. There were 94 children present ranging in ages of 3 months to 5 years, with 19 staff supervising. The inspector was able to view children at free play in both the classroom and playground, observe lunch, and nap time. Toileting and handwashing were also observed. A total of 9 child records and 10 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations. The violations are listed on the violation notice issued to the center and were reviewed with the program director at the exit interview.
Violations
4Evidence:
1) The two year old classroom has four bottles of cleaning material unlocked in a cabinet.
2) The six week to nine month old classroom has three bottles of cleaning material unlocked in a cabinet.
3) The nine month old to one year old classroom has six bottle of cleaning material unlocked in a cabinet.
4) The center director confirmed there were hazardous substances present that were not kept in a locked place.
Evidence:
1) The diaper changing pad on the changing table in the two year old room has a tear, making it an absorbent surface.
2) The center director observed and confirmed the tear.
Evidence:
1) In observing meal time for the 3 year old classroom and 4 year old classroom, there was no staff sitting with the children while eating and drinking.
2)The center director confirmed there was no staff sitting with the children during meal time.
Evidence:
1) The six week to 9 month old classroom does not have an emergency supply of disposable bottles and nipples for the six children currently in care.
2) The nine month old to one year old classroom does not have an emergency supply of disposable bottles, nipples, and commercial formulas for the six children currently in care.
3) The center director confirmed there was no emergency disposable bottles to the two listed classrooms.
