Inspector Notes

An unannounced monitoring inspection was conducted on September 23, 2022. The inspector arrived at 12:40pm and departed at approximately 3:40pm. There were 92 children present and 20 staff members. A sample size of 17 records were reviewed which included 10children?s records and 7 staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, snack time, handwashing, and naptime. First aid kits and emergency preparedness documents were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Standard 8VAC20-780-160-A
Based on record review and interview, it was determined that the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening in which documentation of the screening be submitted at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #2 (date of hire 6/27/2022) had a tuberculosis screening dated 7/28/2022 which is outside of the required timeframe.
2. Staff #1 confirmed that Staff #2 had a tuberculosis screening that was outside of the required timeframe.
Plan of Correction: The center responded with the following: Moving forward the center will ensure that the tuberculosis screening is completed timely, prior to coming in contact with children.
Standard 8VAC20-780-60-A
Based on record review and interview, it was determined that the center did not ensure that for each child enrolled that the work phone number of each parent who has custody is listed to their record.
Evidence:
1. Child #1(date of enrollment 2/14/2022) did not have a work number for their mother.
2. Child #2 (date of enrollment 11/15/2021) did not have a work phone number for their father.
3. Staff #1 confirmed that Child #1 and Child #2 did not have the work phone number of the custodial parent listed to their record.
Plan of Correction: The center responded with the following: During the time of the inspection, Staff #1 obtained the work phone numbers for Child #1 and Child #2 and listed it to their records.
