During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 9/6/24 and completed on 9/24/24 in response to a self-report received by the licensing office on 8/30/24 relating to supervision and special care provisions and emergencies. The inspector reviewed one children?s record and three staff records electronically on September 16, 2024 and September 24, 2024. This inspection included a tour of the facility, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/16/24. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present.
Staff #4 (program director) and record review for Staff #1, Staff #2, and Staff #3 confirmed that the three staff members are not program lead qualified. Staff #1 stated that there was no program lead in the classroom when a serious injury of a child occurred. Staff #2 stated that all three staff members in the classroom at the time were aides and that the lead teacher had left for the day.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all the required information.
The incident report for an injury that occurred on August 30, 2024 did not include the time of the incident and any future action to prevent recurrence of the injury, which are required to be included on the record.
