Inspector Notes

A monitoring inspection was conducted on 12/8/2022. There were 9 children present, ranging in ages from 2 yrs to 4 yrs, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the facility and discussed with the program director during the exit interview.

Based upon review of children's records, the facility has not ensured that each child's record includes the required information about parents.
Evidence:
The record provided for child 2 did not include any information regarding the father.
Plan of Correction: The program director provided the following during the exit interview:
We will ensure that we obtain as much information as possible regarding both parents of each child to assist us in ensuring the safety of each child.
