Inspector Notes

An annual monitoring inspection was initiated on 04/19/2023 and concluded on 04/19/2023. The inspector was on site at the center from approximately 9:40am to 10:50am. There were 8 children in care with 2 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and classroom was inspected today. Five children?s records and two employee records were reviewed during this inspection. Information gathered during the inspection determined areas of noncompliance with applicable regulations or law, and a violation was documented on the violation notice issued to the vendor.

Based on review of documentation and interview, the center did not describe in a written statement to parents or guardians and make available to the general public, aspects of the center's operations concerning physical facilities, enrollment capacity, food services, health requirements for staff and public liability insurance.
Evidence:
The information given to parents upon enrollment did not contain information describing
physical facilities, enrollment capacity, food services, health requirements for staff and
public liability insurance. The center representative was not aware it was a requirement to have this information available in writing.
Plan of Correction: Our plan of action to correct item (1) above is to make copies for each family in JK of the Public Disclosure statement form and the staff qualifications. Distribution will be immediate.
