Inspector Notes

An unannounced, onsite monitoring inspection was initiated on 11/12/2024 and completed on 11/18/2024. The on-site inspection began at 4:50 pm and ended at 5:45 pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 3 staff. The inspector reviewed 5 children?s records during the on-site inspection and 3 staff records during a virtual inspection on 11/18/2024.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A tuberculosis (TB) screening is required every two years from the last screening.

The updated TB screening for Staff 1 was due three months ago. The updated TB screening for Staff 2 was due a month ago.
Plan of Correction: Staff were contacted during the inspection to get the screening updated.
