Inspector Notes

An unannounced, onsite renewal inspection was initiated on 02/05/2025 and completed on 02/07/2025. The on-site inspection began at 3:30 pm and ended at 4:35 pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 2 staff. The inspector reviewed 5 children?s records during the on-site inspection and 2 staff records and 5 applicant records during a virtual inspection on 02/07/2025.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

The updated TB screening for Staff 2 was 2 weeks overdue.
Plan of Correction: The staff has an appointment to go on Monday.
Standard 8VAC20-780-40-E
The center shall maintain compliance with the center?s own policies and procedures that are required by the standards.

The center?s procedures for accounting for all children states that when children are not present, staff will contact the school, parent or emergency contact to ensure that all children are accounted for.

Child 6 (9 years old) was not present on 02/05/2025. The child?s parent arrived to pick up the child. It was determined that the child rode the public-school bus home rather than staying for after school care. The staff had not contacted the school or the parent to ensure that Child 6 was accounted for.
Plan of Correction: Spoke with staff. The staff will check with the office each day when children do not show up for care.
Standard 8VAC20-780-540-E
One working, battery-operated radio shall be maintained in each building used by children.

The battery-operated radio on-site did not work.
Plan of Correction: Sent new batteries to the site.
