Inspector Notes

An unannounced, onsite monitoring inspection was initiated on 08/25/2025 and completed on 08/28/2025. The on-site inspection began at 2:30 pm and ended at 4:05 pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 2 staff. The inspector reviewed 5 children?s records during the on-site inspection and 3 staff records during a virtual inspection on 08/28/2025.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Multiple children, varying in age from 4 years to 10 years, were observed exiting the gymnasium to use a set of restrooms that were in a lobby area. The lobby had a set of doors that was a direct exit out of the center. The children were not within the sight and sound supervision of staff once they entered the lobby.
Plan of Correction: Will speak with the staff and put a reminder in the weekly email.
Standard 8VAC20-780-350-C
When children are in ongoing mixed age groups, the staff-to-child ratio applicable to the youngest child in the group shall apply to the entire group.

There were 23 children (aged 4 years-10 years) in the supervision of 2 staff members. The ratio for 3-year-olds up to school age eligible of 1 staff member for every 10 children was applicable; 3 staff members were required for this group of children.
Plan of Correction: Emailed the staff to remind the staff of notifying admin when they are going to be late.
