During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/19/2025. The on-site inspection began at 12:50pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 34. The inspector reviewed 5 children?s records and 7 caregiver/household member(s) records on-site on 3/19/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the
violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider is required to obtain, for each adult household member ,a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Household Member #1 had a TB screening on file that expired two weeks ago.
Plan of Correction: The household member will get the TB screening this Friday.
Standard 8VAC20-800-210-A
Caregivers are required to obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities, in addition to the first aid and CPR training.

All 4 caregivers (Caregiver #1, #2, #3, & #4) did not obtain a minimum of
sixteen clock hours of training annually for 2024. Each caregiver completed only 2 hours of documented training in 2024.
Plan of Correction: We will make sure that all caregivers take the required 16 hours of training annually moving forward.
