Inspector Notes

A monitoring inspection was initiated and concluded on 05/19/2022. There were 35 children present, ranging in ages from 7 months to 5 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Based on review of three staff records it was determined that not all records contained documentation of a TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff A is dated 10/04/2019.
Plan of Correction: Staff A will obtain an updated TB screening.
Standard 8VAC20-780-270-A
Based on observation of the facility it was determined that not all areas remained in safe and operable condition. Evidence: While observing the playground area the Licensing Inspector observed a tricycle with a cracked seat with sharp edges exposed as well as a crack in the seam of the tug boat play structure posing a pinching hazard.
Plan of Correction: The tricycle was removed during the inspection. The Tug boat will be placed off limits until it can be repaired or removed.

Based on observation of the infant room it was determined that a 1:4 ratio of 1 staff to every 4 children was not always followed. Evidence: There were 2 staff members observed caring for 10 children in the infant room, with children ranging in age from 7 months ? 2 years old.
Plan of Correction: The facility will follow the ratio for the youngest child in a grouping of children.
Standard 8VAC20-780-570-A
Based on observation and interviews it was determined that safety belts were not always used while infants were seated in infant chairs. Evidence: The LI (licensing inspector) observed infant feeding chairs without the protective safety belts. Staff informed the LI that the safety belts were being laundered and that children had utilized the feeding chairs at lunch time without the safety belts.
Plan of Correction: Safety belts will be used for children while seated in infant chairs.
