Inspector Notes
An investigation was initiated from 10:00 a.m. - 12:15 p.m. with local agency Child Protective Services, and Law Enforcement in response to a self reported incident involving behavioral guidance and a forbidden action. Interviews were conducted, documentation, video footage, child and staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 2Standard 8VAC20-780-400-B
Based on interviews and review of video footage it was determined that behavioral guidance was not always constructive in nature and age and stage appropriate. Evidence: On 02/14/2023 at approximately 9:39 a.m. child A (Age 4 years) is seen sitting at a table. Staff reported that the child began to talk and play during "work-time." Staff A is seen roughly grabbing child A by the right arm and moving them away from the table. Child A is then seen crying and moving to the stairs. It was later found at around 5:30 p.m. that child A had three scratch marks on the right inside, underarm.
Plan of Correction: We have retrained individual. Staff has been given a monitor to assist them.
Standard 8VAC20-780-410-1
Based on interviews and review of video footage it was determined that staff engaged in a forbidden action of rough handling a child. Evidence: On 02/14/2023 at approximately 9:39 a.m. child A (Age 4 years) is seen sitting at a table. Staff A is then seen grabbing child A under the right arm and removing them from the table. It was later found at around 5:30 p.m. that child A had three scratch marks on the right inside, under arm.
