During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/24/2025. The on-site inspection began at 9:15am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 80 children present and 11 staff. The inspector reviewed eight children?s records and eight staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The Center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #7, who has been attending seven months, and is under the age of two years of age, does not have additional immunizations on file. The last documentation of immunization is dated 09/18/24.
Plan of Correction: A doctor's appointment is schedule for the child. The information will be added to the record.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. The depth of mulch at the end of two slides chutes measured approximately three inches deep. The height of the slides were over three feet tall. The required depth of mulch is at least six inches. The slides were located on Playground #1.
Plan of Correction: The playground mulch had already been scheduled for a July 2025 delivery. We will re-distribute the mulch to ensure the adequate depth is in the fall zones.
