Inspector Notes
Today an unannounced Monitoring Inspection took place between the hours of 11:15am and 1:15pm. There were 6 classrooms observed with a total of 111 children with 19 staff. The children were observed listening to a story, playing outside on the playground, eating lunch, preparing for rest time and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, 4 emergency medications, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Violations
3Evidence:
1. The green play structure located outside on the playground had multiple areas that are rusted and have sharp edges.
2. The wooden structure above the gardening area on the wall is broken in multiple areas and has a protruding nail sticking out.
3. Multiple silver spikes that are located on the black framing that surrounds the play structures were observed to be protruding from the frame.
Evidence:
1. From approximately 11:42am-11:44am, Staff A was observed to be alone with 22 children. Staff A stated that the assistant teacher was helping one child in the restroom area, which is located outside of the classroom and down the hallway.

1. There was no documentation of a lockdown drill being practiced in 2022 during the time of the inspection.
