Inspector Notes

Today an unannounced Monitoring Inspection took place. There were a total of 111 children with 23 staff within the supervision guidelines. The children were observed playing outside, having circle time, eating lunch and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard 8VAC20-780-160-A
Based on a review of staff records, 2 out of 4 staff did not submit documentation of a negative tuberculosis screening by day 1 of employment or within 30 days prior to the first day of employment.

Evidence:
1. Staff #2 (doh: 1/2/2024) had a tuberculosis screening on file that was dated 5/7/2023.
2. Staff #4 (doh: 8/28/2023) had a tuberculosis screening on file that was dated 9/21/2023.
Plan of Correction: We will make sure all staff have TB screenings within the required timeframe.
Standard 8VAC20-780-560-G
Based on observation, when food is brought from home it is not clearly dated and labeled in a way that identifies the owner.

Evidence:
1. In the Toddler 1 classroom there were 7 lunch boxes observed to have no date for the day on them.
Plan of Correction: We will make sure all food from home is labeled.
