Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/19/2025. The on-site inspection began at 12:15pm and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 147 children present and 22 staff. The inspector reviewed 10 children?s records and 4 staff member records on-site on 11/19/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each staff member is required to submit documentation of a negative tuberculosis
screening.

Staff #1, Staff #3, and Staff #4 did not have documentation of a negative tuberculosis screening within the required timeframe.
Plan of Correction: We will ensure that the staff get TB screenings within the required timeframe moving forward.
