Inspector Notes

Today an unannounced monitoring Inspection took place between the hours of 10:55am and 1:10pm. There were 7 classrooms observed with a total of 85 children with 17 staff within the supervision guidelines. The children were observed eating lunch, transitioning to naptime, participating in a science fair and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Based on record review, staff member tuberculosis screening documentation was not completed within the last 30 calendar days prior to the first date of employment.
Evidence:
1. Staff #1 (date of hire: 5/20/2024) TB screening was dated 4/12/2024.
2. Staff #3 (date of hire: 4/29/2024) TB screening was dated 11/8/2023.
Plan of Correction: Staff have provided updated TB screenings.
