Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 1/15/2025. The on-site inspection began at 1:50 p.m. and ended at 3:45 p.m. The inspector reviewed compliance in the areas listed above. There were 73 children present and 20 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), a tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Every applicant for licensure as a child day center shall undergo a background check every five years.

Agent #2s most recent central registry background check is dated 12/30/2019. The results of a repeat background check have not been obtained.
Plan of Correction: We have completed the request for the results on 12/15/2024 but they have not been received yet. We will contact OBI about the results.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official.

The center has not received a fire inspection from a fire official since 1/11/2023.
Plan of Correction: We have contacted the fire marshalls office about coming to the center to complete an inspection.
Standard 8VAC20-780-280-G
If hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents.

There was an unlabeled spray bottle in the cabinet of classroom 7. Staff in the classroom stated that the bottle was filled with a bleach/water solution.
Plan of Correction: We will make sure all mixed spray bottles are labeled appropriately.
