Inspector Notes

An unannounced inspection was initiated on 4/15/2025 and completed on 4/17/2025 in response to a complaint received by the licensing office on 4/11/2025 relating to allegations of supervision. The inspector reviewed three staff records on-site. This inspection included, document review, tour of the facility, interviews, and observation. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to mandated reporting and are documented on the violation notice issued to the program.

Staff employed by a child day program who have reason to suspect that a child is abused or neglected shall report the matter immediately to the local department's toll-free child abuse and neglect hotline. Staff became aware of a reportable offense, and did not report it to the local department's toll-free child abuse and neglect hotline.
Plan of Correction: Going forward, the center will report any allegations of abuse to CPS immediately.
