Inspector Notes

An unannounced monitoring inspection was conducted on 05/03/2022. There were 11 children, ages 3 years-5 years, and 2 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during outside play, during lunch and during rest time. A total of 5 children?s records and 2 staff records were reviewed. The inspector arrived for the inspection at 11:00 am and departed at 12:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and discussion with staff, the center failed to ensure during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence: There were three children observed laying on their mats during the designated rest period without a bottom cover.
Plan of Correction: Will have parents bring two blankets or a full size sheet.
