Inspector Notes

An unannounced monitoring inspection was conducted on 11/08/2023 and concluded with an electronic record review on 11/09/2023. There were 16 children, ages 3 years- 5 years, and 2 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during arrival, mealtime, teacher directed activities, and self-directed activities. The inspector discussed the following with provider: children?s records. A total of 5 children?s records, and 2 staff records were reviewed. The inspector arrived for the inspection at approximately 8:15 am and departed at approximately 9:45 am.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation, interviews with staff and record review the center failed to inform all staff who work with children of children?s allergies, sensitivities, and dietary restrictions.

Evidence:
1. The record for Child 3 listed ?Dairy? in the Allergies or Intolerance to Food, Medication, etc., and Action to Take in an Emergency section of the child registration form. Staff 1 confirmed they were unaware of this food allergy or restriction. Children were observed having milk and cheese stick for am snack.
2. The record for Child 4 listed ?Lactose Intolerance? in the Allergies or Intolerance to Food, Medication, etc., and Action to Take in an Emergency section of the child registration form. Staff 1 confirmed they were unaware of this food allergy or restriction. Children were observed having milk and cheese stick for am snack.
3. The record for Child 5 listed ?Apple juice, oranges, citrus fruit? in the Allergies or Intolerance to Food, Medication, etc., and Action to Take in an Emergency section of the child registration form. Staff 1 confirmed they were unaware of this food allergy or restriction.
Plan of Correction: Staff will consult with parents and administration staff to gain clarification.
