An unannounced, on-site renewal inspection was initiated on July 17 2024 and completed on July 17, 2024. The on-site inspection began at 10:30am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were80 children present and 12 staff. The inspector reviewed 4 children?s records and 3 staff records on-site]. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Based on record review, the center did not ensure that all agents of the corporation shall undergo a background check every five years from the date of the initial background search.

Evidence:
1) Board Officer #1 and Board Officer #2 records have documentation that a central registry search was conducted on June 17, 2019, but the record does not contain documentation that a follow up search was conducted.
2) Board Officer #2 record has documentation that a criminal record fingerprint check was conducted July 15, 2019, but the record does not contain documentation that a follow up search was conducted
