Inspector Notes
An unannounced monitoring inspection was conducted on 11/30/2022 from 3:50 pm - 5:15 pm. At the time of entrance there were 29 children in care with two staff. The children in care ranged in age from four years - 12 years. Children were observed doing homework, playing games and eating snack in the cafeteria. A fire drill was observed.
Records were reviewed for two children in care. The inspector reviewed three staff records at the Greenbrier North YMCA on 01/10/2023.
Information gathered during the inspection determined non-compliances with applicable standards and violations were documented on the violation notice issued to the program.
Violations
3*The staff-to-child ratio is 1:10 for 3 year olds up to school age eligible.
Evidence:
1. 29 children, ages 4 years - 12 years, were in care with 2 staff at 3:50 pm when the inspection arrived.
a. Three staff were needed to meet the staff-to-child ratio requirement of 1:10 for four year old children.
2. Staff 1 stated that two of the children in care were four years old.
a. The children were all functioning as one group in the cafeteria.
3. Staff 1 confirmed that a 3rd staff was not available.
Centers shall follow the most recent, age appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
1. The snack served to children did not consist of two separate food components as required by USDA.
a. Children were provided with a choice of cheese its or popcorn.
b. Children were allowed to have both choices however, a separate food component from a different food group was not provided.
2. Staff 1 stated that applesauce was available however, this had not been served as the children did not generally like applesauce.
Evidence:
1. The inspector could not locate an enrollment record for child 1 and child 2 in the records file maintained at the after school site.
a. Both of these children were in care during the inspection.
2. Staff 1 stated enrollment records were not available for child 1 and child 2.
