Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 12/10/2024 and completed on 12/11/2024. The on-site inspection began at 4:34 pm and ended at 5:35 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 4 staff. The inspector reviewed 5 children's records on-site. On 12/11/2024 the inspector reviewed 3 staff records on-site at the Greenbrier North YMCA where staff records are maintained.

Standard 8VAC20-780-160-A
Documentation of the tuberculosis screening shall be submitted at the time of employment and shall have been completed within the last 30 calendar days of the date of employment.

The tuberculosis screening for staff 1 was completed more than 30 days after the first date of employment.
Plan of Correction: Administrative staff stated that all TB screenings will be obtained prior to employment going forward. It will be ensured that the screening was completed within 30 days prior to employment.
