Inspector Notes

A monitoring inspection was conducted on 3/11/2024 from 3:30pm to 5:00pm. At the time of the inspection there were 49 children in care and 5 staff members. Children were observed participating in transition activities and a group game. Records were reviewed for 7 children while at the facility. Five staff records were reviewed at the central office. The inspector toured the facility and observed supervision. Evacuation drills, emergency supplies, medications and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on observation and interviews, it was determined that the facility did not ensure the license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
Evidence:
1.Upon arrival to the center at 3:30pm, the parent board containing all required postings was not out and available for public viewing.
2.The most recent license was not posted. The license posted was a provisional license and had expired 9/2023.
3.The most recent inspection summary dated 9/14/24 was not posted. The inspection summary posted was dated 5/2023.
4.Staff #1 confirmed the parent board had not been set up at the correct time and the documents were not current.
Plan of Correction: During the inspection the parent board was set out. The current license and inspection summary have been posted.
Standard 8VAC20-780-240-A
Based on the review of records and interview, it was determined the center did not ensure that the Virginia Department of Education (VDOE) sponsored orientation course is completed within 90 calendar days of employment.
Evidence:
1.The record for staff #3 did not contain documentation that the VDOE 10-hour orientation training had been completed.
2.Staff #6 confirmed the training certificate was not in the record.
Plan of Correction: The VDOE orientation training has been completed by staff #3 and the certificate has been placed in the staff record.
