Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/17/24. The on-site inspection began at 3:00pm and ended at 5:00pm. The inspector reviewed compliance in the areas listed above. There were 52 children present and 3 staff. The inspector reviewed 6 children?s records and 4 staff records at a central location on 10/18/24. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/28/24. A POC submitted after this date will not appear on the public website.

At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow up tuberculosis (TB) screening.
Evidence:
The two year follow up TB screening for staff #2 was 2 months overdue. The updated TB screening was not completed.

Repeat Violation
Staff shall not allow a child to leave the center unsupervised.
During the inspection, child #6 opened the exit door and walked out of the facility unsupervised.

The required ratio for children school age eligible up to 9 years is one staff per eighteen children.
Staff #1 and staff #2 were observed supervising thirty-nine children when transitioning from the cafeteria to the bathrooms. Based on the number and ages of the children, three staff members were required.
