Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/20/25 and completed on 3/21/25. The on-site inspection began at 3:15pmand ended at 5:15pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 36 children present and 4 staff. The inspector reviewed 5 children?s records on site and 5 staff records electronically on 3/21/25. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/4/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center. The medication authorization, signed by a parent for Child #1 had expired 18 months ago. The medication had not been picked up and was not disposed of.
Plan of Correction: Expired medication for Child #1. The Youth Development Director will attend medication Administration training on Friday, April 11, 2025.
