An unannounced monitoring inspection was initiated and concluded on June 8, 2023. This monitoring inspection was a follow up to the Intensive Plan of Correction previously developed by the center There were 90 children in care with 16 staff and administrators. Classroom observations were made, and children were observed in teacher
and self-directed activities. Violations were cited in the areas of Administration, Special Care Provisions and Emergencies and Background checks. These violations appear on the violation notice and were discussed with the program administrator during the exit interview at the conclusion of the inspection.

Based on record review and interview the center failed to ensure that background checks are completed as required.

Based on observations and interviews the center failed to confidentially maintain a current written list of all children's allergies, sensitivities and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. Additionally, The list was not dated and kept in each room or area where the children are present.
