Inspector Notes
An unannounced on-site inspection was initiated and completed on 1/24/25 from 12:00 until 1:15. The inspector reviewed compliance in the areas listed above. There were 15 children and 3 staff present. The inspector reviewed 4 children?s records. Three staff records were electronically reviewed on 1/23/25. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
