Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9/25/24 and concluded on the same day., as a part of licensure period. The on-site inspection began at 10:15 and ended at 11:25. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregiver; the point total was 17. The inspector reviewed 3 children?s records and 2 staff records . This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Records for 2 of the 3 children's records reviewed were missing information. Child 2 did not have father employer listed and Child 3 did not have a second emergency contact.
