During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 3/25/26 and completed on the same day, as a part of licensure period. The on-site inspection began at 10:15 and ended at 11:25. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 22. The provider arrived with her two children at 10:45 adding 2 points.The inspector reviewed 5 children?s records and 4 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Background checks must be conducted every 5 years.

One staff, Staff 1, is missing a subsequent Sworn disclosure statement.
Plan of Correction: I will fill out a new sworn disclosure statement.
Standard 8VAC20-800-140-D
A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works

The substitute provider did not record the hours worked as a substitute for 3/24/26.
Plan of Correction: I will fix that today and talk to the staff to make sure it is filled out.
Standard 8VAC20-800-220-A
To safely perform medication administration practices listed in 8VAC20-800-710 whenever the family day home has agreed to administer prescription medications or nonprescription medications, the administration shall be performed by a caregiver who has the required MAT training.

A child, child5, is on a medication that is not included in the basic MAT training. Not on the staff currently has the additional training. No caregivers are authorized to give the medication for the child.
Plan of Correction: We will take the additional MAT training required to administer the medication.

A child's records must include a allergy action plan, ig the child have an allergy that requires medication.

One child, child 5, is missing a completed allergy action plan for the medication that is at the family day home for his allergies.
Plan of Correction: I will ask the parents for a new allergy action plan.
