Inspector Notes

An unannounced renewal inspection was conducted on February 13, 2023 from approximately 10:15 am to 12:30 pm. There were 66 children in attendance and a total of 12 staff present. 5 staff records and 7 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed playing in centers, eating lunch and playing outside, while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 02/24/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard 8VAC20-780-140-A
Based on a review of children?s records the center failed to ensure that each child shall have a physical examination within 30 days after the first day of attendance. Evidence: The record for child #5, date of enrollment 06/20/22, contained a physical dated 08/22/22.

Standard 8VAC20-780-350-Q
Based on records review, observation, and interview, the center failed to ensure a parent?s written permission and a written assessment by the program director and program leader, prior to assigning a child to a different age group.
Evidence: 1) Classroom #3 had 10 children ages 16-24 months with 2 staff supervising. The record for child #1, 15 months, determined that the child be placed in a 1:4 staff-to-children ratio. When interviewed, the Director stated that the child?s parent wanted them assigned to this age group. There was no written permission in the record for child #1 to be placed in a different age group.
2) Classroom #4 had 7 children, ages 24-36 months, with 1 staff supervising. The record for child #5, 17 months, determined that the child be placed in a 1:5 staff-to-children ratio. When interviewed, the Director stated that the child?s parent wanted them assigned to this age group. There was no written permission in the record for child #4 to be placed in a different age group.
3) Classroom #8 had 10 children, ages 3 to 5 years, with 1 staff supervising. The record for child #2, 35 months old, determined that the child be placed in a 1:8 staff-to-children ratio. When interviewed, the Director stated that the child?s parent wanted them assigned to this age group. There was no written permission in the record for child #2 to be placed in a different age group.

Based on a review of staff records, the center failed to obtain documentation that the individual possesses the education, certification, and experience required by the job position. Evidence: The record for staff #4, date of employment 12/13/22, did not contain documentation of education or certification required by the job position. Staff #4 was observed working alone in a classroom with a group of children.
