Inspector Notes

A complaint inspection was initiated on May 24, 2024 in response to a complaint that was received on April 29, 2024 in reference to ratios, diaper changing procedures, and emergency evacuation procedures.. Documentation was gathered, records were reviewed, and interviews were conducted on May 24th from approximately 10:00 am to 1:15 pm and again on June 5th from 10:45 am to 12:00 pm. The preponderance of evidence gathered during the investigation supports a violation of one or more standards in regard to the complaint. Therefore the complaint is considered to be valid.

Violations were cited.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 06/25/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on interviews, the center failed to ensure that children younger than three years shall have their diapers changed on a changing table or countertop designated for changing.
Evidence: Interviews with staff revealed that children under the age of three have their diapers changed while they are standing in the bathroom.
Plan of Correction: Moving forward students in our two year old classroom Stepping Stones will be changed on the changing table, escorted to the bathroom and placed back on the changing table during diaper changing times.
