Inspector Notes

An unannounced inspection was initiated on 05/02/2025 and completed on 05/12/2025 in response to a complaint received by the licensing office on 04/23/2025 relating to allegations of operational responsibilities, parental involvement, and staff training. The inspector reviewed 1 children?s record on-site and electronically on 05/12/2025. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to children's records and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 05/23/2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain a written care plan for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

The record for child #1, enrolled for 3 days, did not contain a written care plan that included instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Plan of Correction: There was a care plan completed with the parent's signature, but it was missing the physicians signature. Forms will be reviewed thoroughly before allowing students to start to ensure all the proper signatures are in place.
