During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
This inspection was amended on 07/29/25 to correct a typographical error in violation 8VAC20-780-(2)-60-A-8.
An unannounced, on-site monitoring inspection was initiated on 07/08/2025 and completed on 07/22/2025. The on-site inspection began at 10:44 am and ended at 2:34 pm. The inspector reviewed compliance in the areas listed above. There were 104 children present and 19 staff. The inspector reviewed 11 children?s records and 10 staff records on-site and electronically on 07/22/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 07/29/2025. A POC submitted after this date will not appear on the public website.
Violations
3The allergy list did not contain child #4, enrolled for 2 years, who has a diagnosed food allergy.
Staff #5 and staff #6 were in classroom #9 with a group of children. Staff #5 and staff #6 do not have current certification in first and and CPR.
The center is required to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
1. Child #4, enrolled for 2 years, has a diagnosed food allergy. The record for child #4 did not contain a written care plan with instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
2. Child #9, enrolled for 17 months, has a diagnosed food allergy. The record for child #9 did not contain a written care plan with instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
The center suspended services for child #9 until an updated written care plan was received by the physician. An updated written care plan was received on 7/17/25.
