Inspector Notes
An unannounced monitoring inspection was conducted on Septemeber 28, 2022 from approximately 1:30pm-3:2 pm. There were 20 children present at this time with 3 staff members. Children were observed during rest time. Three staff records and three children's records.
Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-inplace drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on
the violation notice issued to the program.

Evidence:
Staff #2 hire date 06/28/2022 tuberculosis screening was completed 03/10/2022 which was more than 30days prior to employment.
Evidence:
Staff #1 and Staff #2 were missing the following components in there orientation :
1- Prevention of sudden infant death syndrome and use of safe sleep practices;
2- Prevention of shaken baby syndrome and abusive head trauma, including
procedures to cope with crying babies or distraught children
3- Prevention of and response to emergencies due to food and other allergic
reactions including:
a. Recognizing the symptoms of an allergic reaction;
b. Responding to allergic reactions;
c. Preventing exposure to the specific food and other substances to which the
child is allergic; and
d. Preventing cross contamination; and
4- Prevention and control of disease
Director confirmed the orientaiton components are missing.

Evidence: Confirmed with the owner/director the center did not have gauze pad and antiseptic wipes/solution.
