Inspector Notes

An unannounced monitoring inspection was conducted on Septemeber 28, 2022 from approximately 1:30pm-3:2 pm. There were 20 children present at this time with 3 staff members. Children were observed during rest time. Three staff records and three children's records.
Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-inplace drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on
the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on record review and staff interview, the licensee did not ensure documentation of the screening was submitted at the time of employment and prior to coming into contact with children and documentation was completed within the last 30 calendar days of
the date of employment.

Evidence:
Staff #2 hire date 06/28/2022 tuberculosis screening was completed 03/10/2022 which was more than 30days prior to employment.

Standard 8VAC20-780-240-C
Based on staff interview and record reveiw, the licensee did not ensure all components in orientation were included in staff orientaiton training.

Evidence:
Staff #1 and Staff #2 were missing the following components in there orientation :
1- Prevention of sudden infant death syndrome and use of safe sleep practices;
2- Prevention of shaken baby syndrome and abusive head trauma, including
procedures to cope with crying babies or distraught children
3- Prevention of and response to emergencies due to food and other allergic
reactions including:
a. Recognizing the symptoms of an allergic reaction;
b. Responding to allergic reactions;
c. Preventing exposure to the specific food and other substances to which the
child is allergic; and
d. Preventing cross contamination; and
4- Prevention and control of disease
