Inspector Notes

A subsidy health and safety (SHSI) inspection was completed. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. The children were observed getting ready for nap time and during nap time. The inspection started at 11:45 am and concluded at 1 pm.

Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Based on record review and staff interview the vendor did not ensure Staff who work directly with children shall annually complete the departments health and safety update course.

Evidence: Staff #2, date of hire 5/18/2020 and date of VA Pre-Service training 5/15/2020 did not have documentation for their 2023 health and safety update course.
Plan of Correction: The staff member completed the training on 2/28/2024.
