Inspector Notes
An unannounced monitoring inspection was conducted on 08/10/2023 from 10:30 am-12:55 pm. There were 81 children supervised by 12 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play and tabletop activities. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations
3Evidence:
1. In the toddler classroom, there were 2 children in care who were 18 months old and the other 24 children were over the age of 18 months old.
2. Since the children aged 18 months were in care the ratio required is 1 staff to 5 children.
3. There was 4 staff with 26 children during the inspection.
4. The ratio of the youngest age must be followed in a mixed-age grouping such as this.
Evidence: Child #2 has emergency medication that is missing a prescription medication label that includes the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
1. In the Pre-K/Kindergarten Classroom, 10 out of 10 lunch boxes were not dated.
2. In the Pre-Primary Classroom, 7 out of 7 lunch boxes were not dated.
