Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and reviewed video footage.

A complaint inspection was initiated on July 27, 2021 and concluded on September 30, 2021. A complaint was received by the department regarding allegations in the areas of programs. The staff in charge was contacted by telephone to conduct the investigation. The inspector interviewed, reviewed video footage and reviewed documentation required to complete the investigation. There was not a preponderance of evidence gathered during the investigation to support the allegation(s) the allegation is deemed not valid.
