Inspector Notes

An unannounced complaint inspection and a report of an injury to a child while under supervision of the facility that required outside medical treatment was conducted on May 6, 2022. The complainant reported concerns regarding staffing and supervision and special care provision and emergencies. An inspection of the facility was conducted, documentation was reviewed and interview was held with staff. Based on gathered information, there was no violations cited related to information gathered during the investigation of a report of an injury to a child while under the supervision of the facility that required outside medical treatment.
