Inspector Notes

An unannounced complaint inspection was conducted on 03/22/2023 in response to a complaint received regarding special care provision and emergencies and special services.

Based on the information gathered which included staff interviews and observaton this complaint is found to be valid in special care provision and emergencies and special services.

Violations were found in the areas of special care provisions and emergencies.

Standard 8VAC20-780-270-A
Based on record and staff interview, the licensee did not ensure areas of the center were maintained safe and operable.

Evidence:
In the infant classroom, the closet door will not close causing a possible pinching hazard.
Plan of Correction: The door in the infant room has been fixed.
Standard 8VAC20-780-570-A
Based on staff interview, the licensee did not ensure when high chairs or feeding infant seats are used the protective belt shall be fastened securely.

Evidence: Infant room staff confirmed the three high chairs/ feeding table are used. Two of three high chairs were missing bottom strap that is attached to the seat. Confirmed with staff.
Plan of Correction: We have replaced both high chairs in the infant room .
