During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If
there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 9/18/2025 and completed on 9/18/2025 with a Arlington County Child Care office in response to a complaint received by the licensing office on 9/15/2025 relating to allegations of building maintenance and pest control.This inspection included tour of the facility, interviews, observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Though not a part of the original complaint, violations that were not reported were found related to physical plant and are documented on the violation notice issued to the program.

Areas and equipment of the center, inside and outside, shall be maintained in a clean and safe condition.

Rodent feces were observed in a closet located in a 2-3 year old classroom. This area was not accessible to children.
Plan of Correction: Staff have implemented proper CDC cleaning guidelines to be conducted everyday. Staff has contacted pest control and have received proper guidance on removing rodent feces and center improvements. Staff has worked alongside the health department on implementing changes to the building to assist with keeping the rodent away.

Hazardous substances such as cleaning materials and insecticides shall be kept in a locked place using a safe locking method that prevents access by children.

1.In a two year old classroom, cleaning wipes were left unlocked and accessible to children in the diapering area. Cleaning material was found in a children's bathroom floor between a 2-3 year old classroom. The supply closest in the hallway was also unlocked and contained cleaning agents.
2. Insecticides were found in a bathroom used by 2-3 year old children in a toddler classroom and were accessible to children in care.
Plan of Correction: Staff will ensure that all cleaning and hazardous materials are safely stored and out of reach of children. Staff will ensure all cabinets and doors are locked at all times using the child safety locks provided. To ensure the safety of the children. Staff will provide a child safety gate and ensure it is placed in the doorway of all classroom bathrooms. Staff will continue to maintain supervision to all children
at all times when the bathroom is in use and not in use.
