An unannounced monitoring inspection was conducted on August 30, 2023. This on-site visit was conducted from 10:50 a.m. through 11:30 a.m. Upon arrival, there were 17 children, ages 4 through 45 in the care of 4 staff members. Five children and four staff records were reviewed.

Based on review of staff records the center failed to obtain an updated Virginia Central Registry finding for one staff member.
Evidence:
1. Four staff records were reviewed.
2. Staff #2 did not have documentation of obtaining an updated Virginia Central Registry Search within five years of the previous check.
3. Staff 2 Virginia Central Registry check was documented 12/20/2015.
4. Administration confirmed an updated Virginia Central Registry check had not been obtained within 5 years of the previous one for Staff #2.
