Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone/email contacts, documents review, interviews and a tour of the program. A monitoring inspection was initiated on 8/9 and concluded on 8/23. The provider was contacted by email and then a virtual inspection was conducted. There were 9 children present, ranging in ages from 15 months to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of 2 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review, 1 out of 2 children's records did not include all required information.
Evidence
1. Child #1 does not have the insurance policy number included on the child's registration form.
