Inspector Notes
An unannounced monitoring inspection was conducted on 10/12/22. The provider and another caregiver was present with 13 children , a total of 36 points. The children were observed eating lunch and getting ready for nap time as well as nap time. The children had Mac and cheese, salad and fruit for lunch. Interaction between caregivers and children observed during the inspection. A violation notice was given to the provider along with this inspection summary. 6Evidence: Reviewed 4 children's records, Child 1 is missing a physical.
Evidence:Based on the review of the providers training, the provider does not have a current CPR certificate available for view.
The two assistants both have CPR certification that has expired.
Caregiver 2 expired 10/7/22

Based on the review of the providers training, the provider could not show current certification in first aid.
The first aid certificates for caregiver 2 and 3 are expired. Staff 2 expired 10/7/22 and staff 3 expired 8/25/2022
