The Department requested that the provider complete an Intensive Plan of Correction (IPOC) as result of the last inspection related to background checks. During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site focus inspection was conducted on 05/05/2025 from 11:40am - 12:55pm as part of an Intensive Plan of Correction (IPOC). The inspector reviewed 6 staff records on-site. The inspection included document review and interview.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain a central registry check (CPS) for each employee within 30 days of employment.

The record for Staff #1, employed for almost two years, did not have results of a central registry check.
Plan of Correction: The center contacted OBI to obtain the results from the CPS request submitted in 2023. The application/results could not be found. The center submitted a new online request on 5/7/2025 and will follow up if results are not received within 30 days.
