Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 11, 2024. The on-site inspection began at 10:15am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was11. The inspector reviewed 3 children's records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation and interview, documentation of an orientation was not signed and dated by the provider and the provider and assistant.
Evidence:
1. The record for Caregiver #2 did not contain signed documentation of orientation.
2. Caregiver #1 acknowledged that Caregiver #2 had an orientation but did not sign the document. Caregiver #1 and Caregiver #2 signed the documentation of orientation at the time of inspection.
Plan of Correction: The orientation document has been signed and dated by Caregiver #2.
